Rate a Rhazes answer good or bad, and say how serious a problem was, so a wording slip is handled differently from a clinical risk.

Every response carries Good response and Bad response.
Reporting a bad one
Bad response opens a short form asking how serious the problem was, on a scale from a very minor slip unlikely to cause problems through to something critical that needs urgent correction. There is room for a comment as well.
Saying how serious it was matters more than it looks. It is what separates a wording preference from an answer that could lead to a clinical mistake, and the two get handled very differently.
Where it goes
Feedback goes to the team improving Rhazes. It is not visible to other users.
