Where it is
The Clinical information tab on a patient. It is a plain editor, and it saves as you type.
What to put in it
Relevant clinical data, medical history and demographics. In practice the useful thing is whatever you would otherwise have to re-explain: the background, the current problem, what was decided last time, what is planned next.
How it gets used
This is what Rhazes reads when the patient is linked to a chat. Anything in here is available as context to whatever you ask.
That cuts both ways. Detail that helps you will help Rhazes; a record that is out of date will quietly push a chat in the wrong direction, so it is worth keeping current rather than writing once.
