    Dear Dr Vance,

Re: Referral concerning Arthur Pendelton, 14/05/1958

I am seeking your clinic's further assessment and management of Arthur Pendelton regarding unexplained iron deficiency anaemia and altered bowel habits.

    Clinical Details

Presenting Complaint: The patient presents with progressive fatigue and increased stool frequency with occasional dark stools.

Duration: These symptoms have been present for the past six weeks.

Relevant Findings: Examination revealed pale conjunctivae, a soft and non-tender abdomen with no palpable masses, and a PR exam showing brown stool with no visible masses or haemorrhoids. His BP is 128/78 mmHg and HR is 82 bpm.

Past Medical History: The patient has a history of hypertension and osteoarthritis of the knees.

Current Medications: Current medications include perindopril (Coversyl) 5 mg PO daily and paracetamol (Panadol Osteo) 1,330 mg PO TID PRN.

    Investigations

Recent Tests: Recent investigations include a full blood examination, iron studies, and a faecal occult blood test.

Results: Results demonstrated a haemoglobin of 102 g/L, MCV of 74 fL, ferritin of 12 ug/L, and a positive immunochemical faecal occult blood test on two separate samples.

    Reason for Referral

In view of his symptomatic iron deficiency anaemia and positive faecal occult blood test requiring urgent endoscopic evaluation to exclude colorectal malignancy, I would value your specialist opinion and recommendations for this patient's care.

    Patient's Contact Information

Phone Number: The patient can be contacted on 0412 345 678.

Email Address: The patient's email address is a.pendelton58@email.com.au.

The following materials are enclosed for your consideration: pathology reports from Melbourne Pathology dated 20/10/2023 and a current patient health summary. Please contact me if any additional information would be helpful.

Thank you for considering this referral. I would welcome your assessment and advice.

Yours sincerely, Dr Sarah Jenkins General Practitioner Phone: (03) 9555 1234, Fax: (03) 9555 1235 Bayside Family Medical Centre
