This report is prepared on 15/10/2023, at the request of WorkSafe Victoria, to provide a comprehensive medical account for Marcus Vance, born on 12/04/1982. It has been written by Dr Sarah Jenkins, MBBS, FRACGP, a healthcare professional with 14 years of experience. I have treated Marcus Vance for 5 years and act as their regular healthcare provider.
The purpose of this report is to assess the current medical condition of Marcus Vance, who has experienced an L4/L5 disc prolapse with right-sided radiculopathy following an injury sustained on 02/08/2023. The referral for this medical assessment was made by Allianz Australia Workers' Compensation.
Marcus Vance has a history of mild asthma and episodic mechanical lower back pain in 2018, which resolved completely with conservative management. They have also undergone a laparoscopic appendicectomy in 2010, and relevant hereditary conditions in the family include maternal osteoarthritis and type 2 diabetes mellitus. These factors may contribute to the current condition and should be considered in the overall medical context.
In relation to the current injury or medical event, Marcus Vance experienced an acute onset of severe lower back pain while lifting a heavy crate of automotive parts at work on 02/08/2023. The injury mechanism involved heavy lifting coupled with awkward lumbar flexion and rotational strain, such as a slip, fall, or repetitive strain. Immediately after the incident, the patient reported symptoms including an audible pop in the lumbar region followed by immediate, sharp, debilitating pain, involving paraspinal muscle spasm and shooting pain radiating down the posterior aspect of the right leg such as pain, swelling, or discomfort.
On medical examination, Marcus Vance reported persistent, severe lower back ache, including paraesthesia along the right L5 dermatome and weakness in the right foot. Physical findings were an antalgic gait, significant paraspinal tenderness at the L4/L5 level, lumbar flexion restricted to 30 degrees, and a positive right straight leg raise test at 40 degrees, such as tenderness, swelling, restricted range of movement, or other observations. Diagnostic imaging, including a lumbar spine MRI performed on 10/08/2023, was performed and showed a moderate-to-large right paracentral disc extrusion at L4/L5 impinging the traversing right L5 nerve root. Laboratory tests were also undertaken, with results of FBE, UEC, and CRP all within normal reference ranges, excluding underlying inflammatory or infectious pathology.
The initial treatment given to Marcus Vance comprised brief relative rest, heat application, and acute pharmacological analgesia, including ibuprofen (Nurofen®) 400 mg PO TID and paracetamol (Panadol®) 1,000 mg PO QID. Ongoing treatment has comprised structured multimodal rehabilitation, such as twice-weekly physiotherapy focusing on core stabilisation, hydrotherapy, and neuropathic pain management. The patient has been prescribed pregabalin (Lyrica®) 75 mg PO BID and meloxicam (Mobic®) 15 mg PO daily, with specified dosages and frequencies. Referrals to Mr David Chen (Neurosurgeon) and a workplace occupational rehabilitation provider have also been made, and follow-up care includes regular fortnightly GP reviews to monitor neurological status and work capacity, with appointments scheduled for 29/10/2023 and 12/11/2023.
In terms of progress, Marcus Vance has demonstrated slow but steady symptomatic improvement, including a reduction in resting radicular leg pain and improved sleep quality. However, the patient continues to have significant mechanical lumbar intolerance, including difficulty with prolonged sitting (> 20 minutes), standing (> 30 minutes), bending, twisting, and lifting weights exceeding 5 kg, affecting their ability to perform their pre-injury duties as a warehouse storeperson and domestic gardening tasks. Pain and disability assessments show an Oswestry Disability Index (ODI) score of 44% (severe disability), with the patient reporting a visual analogue scale (VAS) pain severity of 5/10 at rest, escalating to 8/10 with physical exertion.
Looking ahead, the prognosis for Marcus Vance is guardedly favourable, with recovery expected to be prolonged over a 6 to 12 month timeframe. Continued recovery is expected to require a graduated return-to-work program and ongoing conservative management, including targeted clinical Pilates, gradual weaning of pregabalin, and regular ergonomic monitoring. If the condition fails to improve or deteriorates, possible future interventions such as a CT-guided right L5 transforaminal epidural steroid injection or surgical microdiscectomy may be required.
In my medical opinion, the current condition of Marcus Vance is directly attributable to the workplace lifting incident on 02/08/2023 and remains subacute, significantly affecting their ability to perform manual handling tasks or participate in unrestricted daily life. Long-term treatment and workplace accommodations are required to maintain a stable prognosis.
In summary, the findings show that Marcus Vance is suffering from a work-related L4/L5 disc prolapse with right L5 radiculopathy, currently unfit for pre-injury duties but suitable for modified sedentary duties, with the key points being MRI-confirmed nerve root impingement, ongoing functional limitations, and a requirement for strict lifting restrictions (< 5 kg). I recommend continuation of physiotherapy, commencement of a graduated return-to-work plan on light duties (4 hours/day, 3 days/week), and pending neurosurgical review, including additional treatment, rehabilitation, or referral to specialists.
I, Dr Sarah Jenkins, certify that the information in this report is accurate and complete to the best of my knowledge and belief. This report is based on the patient’s medical records and findings from medical examinations and treatment sessions.
Signed: Dr Sarah Jenkins
