Practice Name: Yarra Valley Medical Centre Contact Number: (03) 9123 4567

    Certificate Issue Date: 24/10/2023

    To Whom It May Concern

This document confirms that David John Reynolds has received care from me and underwent medical assessment on 24/10/2023. After treatment and clinical review, it is my professional judgment that the person is fit to resume employment from 25/10/2023.

The patient is sufficiently recovered to undertake their normal work duties unless a different arrangement is stated. Where relevant, temporary adjustments or limitations are set out below to help facilitate a safe and lasting return to the workplace.

    Work Modifications or Restrictions

The patient requires modified duties for a period of two weeks following their return to work due to a resolving lumbar strain. Specifically, they must avoid lifting, pushing, or pulling loads greater than 5 kg, and they should avoid repetitive bending or twisting of the lumbar spine. Furthermore, they require a 10-minute postural break every hour to alternate between sitting and standing, and their shifts should not exceed 6 hours per day during this initial two-week transition period.

    Follow-Up Plan

A clinical review has been scheduled in two weeks on 07/11/2023 to reassess functional capacity and oversee the return-to-work progression, with the aim of clearing the patient for pre-injury duties. In the interim, the patient has been advised to continue twice-weekly physiotherapy sessions and adhere to their prescribed home exercise programme for ongoing physical rehabilitation.
