      • Summary of Discussion: Pt had 2 falls at home in past month. Sitting BP 135/80 mmHg, standing BP 110/65 mmHg. OT home assessment identified trip hazards (loose rugs) and lack of grab rails in bathroom.
    • Polypharmacy and medication review

      • Discussed by: David Chen, Accredited Practising Pharmacist
      • Summary of Discussion: Current regimen includes perindopril (Coversyl®) 5 mg PO daily, amlodipine (Norvasc®) 5 mg PO daily, and temazepam (Temaze®) 10 mg PO nocte. Agreed that antihypertensives and nocturnal benzodiazepine are contributing to falls risk.
    • Declining mobility and ADLs

      • Discussed by: Sarah Jenkins, Aged Care Nurse Practitioner
      • Summary of Discussion: Increasing difficulty with showering and meal preparation. Requires Level 3 Home Care Package (HCP) upgrade to support safe ageing in place.

    MBS Billing

    • MBS Item 735
    • 25 minutes duration; multidisciplinary case conference organised and coordinated by GP to align falls prevention strategy and medication rationalisation.

    Care Plan

    • Cease amlodipine (Norvasc®) 5 mg PO daily immediately; continue perindopril (Coversyl®) 5 mg PO daily and monitor BP.
    • Wean temazepam (Temaze®) to 5 mg PO nocte for 2 weeks, then cease.
    • OT to arrange installation of bathroom grab rails, shower chair, and removal of loose rugs.
    • NP to submit ACAT reassessment request for Level 3 HCP upgrade.

    Follow-Up

    • GP review in 2 weeks for repeat postural BP check and sleep review.
    • Next Conference Date: 23/01/2024
