I am writing to confirm that Arthur is presently receiving care from me at Oakham Medical Practice for an acute exacerbation of chronic obstructive pulmonary disease (COPD). He is currently managed with prednisolone 30 mg PO OD and doxycycline 100 mg PO OD, alongside salbutamol 100 mcg INH PRN.
This letter has been provided at the patient's request to confirm his current medical status for his employer. Due to marked shortness of breath and fatigue, he remains unfit for work from 22/05/2024 to 05/06/2024 inclusive, and a follow-up review is scheduled prior to his return.
