SOAP Note
Subjective
- Dynamics within the family: Pt (Sarah Jenkins, 36yo F, DOB: 12/05/1988, MRN: 492-817-04) reports increased tension at home following a recent visit from her mother. Notes feeling infantilized and undermined in front of her spouse and children.
- Relationships with members of the extended family: Discussed ongoing enmeshment with maternal aunt, who frequently triangulates pt into maternal conflicts via unsolicited phone calls.
- Emotional processing related to family concerns: Expressed pervasive guilt and somatic anxiety (chest tightness) when attempting to limit contact with her mother, linking this to childhood fears of maternal emotional withdrawal.
- Reflections regarding personal relationships: Recognized a parallel pattern in her marriage where she over-functions to prevent her husband's perceived displeasure, mirroring her role as the family peacemaker.
- Conversations about physical intimacy and boundaries: Reported low libido and avoidance of physical intimacy with her husband. Identifies feeling "touched out" and equates sexual expectations with a loss of bodily autonomy.
- Difficulties related to parenting: Struggles with setting firm limits with her 6yo son's bedtime tantrums, fearing that enforcing discipline makes her "cold and authoritarian" like her mother.
Objective
- Ability to participate in a constructive discussion: Engaged, cooperative, and maintained appropriate eye contact throughout the 50-minute session (14:00–14:50 CST, 15/10/2023). Speech normal in rate, rhythm, and volume. Affect constricted initially, brightening slightly when intellectualizing, and tearful when discussing maternal guilt.
- Degree of insight into personal and family dynamics: Demonstrates emerging insight into intergenerational patterns, specifically how her fear of maternal rejection drives her current people-pleasing behaviors.
- Capacity for introspection: High capacity for self-reflection; tolerated interpretations regarding her defenses (intellectualization, reaction formation) without becoming overly defensive.
- Ability to express boundaries clearly: Articulates the need for boundaries intellectually in session, though tone becomes hesitant and apologetic when role-playing direct limit-setting.
Assessment
- Description of dynamics within the family system: Family of origin is characterized by enmeshment and diffuse boundaries, where autonomy is historically framed as betrayal. Current nuclear family system shows compensatory over-accommodation by the pt.
- Recognition of attachment-related themes: Displays anxious-preoccupied attachment traits. Hypervigilant to cues of abandonment or disapproval from attachment figures, leading to chronic self-effacement.
- Examination of recurring relationship patterns: Core conflictual relationship theme involves a wish to be autonomous and accepted, met with an anticipated response of rejection from others, resulting in a submissive, caretaking response from the self (and subsequent resentment/withdrawal from intimacy).
- Assessment of the client's progress and development: Pt is making steady progress in moving from somatic/anxious symptom reporting to psychological mindedness. Beginning to link childhood defenses to current marital and parenting distress. Continues escitalopram (Lexapro®) 10 mg PO QAM with partial reduction in baseline GAD symptoms; no side effects reported.
Plan
- Ongoing examination of specific themes: Continue psychodynamic exploration of the "good daughter" ego ideal and the unconscious equation of assertiveness with aggression.
- Processing family dynamics and their effects: Process the grief and anger associated with recognizing her mother's emotional limitations. Monitor transference for compliance/people-pleasing within the therapeutic alliance.
- Formulation of communication strategies: Develop concise, non-defensive verbal responses to de-escalate triangulation attempts by extended family.
- Assistance with establishing boundaries: Support pt in tolerating the affective discomfort (guilt) that follows setting a firm boundary with her mother and her 6yo son.
- Possible use of further therapeutic exercises or techniques: Utilize free association and dream analysis if resistance increases. Continue weekly 50-minute individual psychodynamic psychotherapy (next session 22/10/2023 at 14:00 CST) and maintain escitalopram 10 mg PO QAM.
