Re: Progress Review Under Medicare Better Access Initiative

    Patient Name: Sarah Jenkins

    Date of Birth: 19/08/1988

    Medicare Number: 2948 57102 3 (Ref: 1)

    Dear Dr Chen,

I am writing to provide a progress review for Sarah Jenkins following their participation in psychological therapy under the Medicare Better Access Initiative. Sarah Jenkins's latest DASS-21 result placed her in the mild range for depression, moderate range for anxiety, and mild range for stress.

Ms Jenkins initially presented with generalised anxiety, occupational burnout, and sleep-onset insomnia triggered by workplace restructuring. Clinical assessment confirmed GAD with secondary depressive symptoms, currently managed concurrently with escitalopram (Lexapro®) 10 mg PO OD.

    • Symptom reduction: Panic episodes have decreased from 3x/week to zero over the past fortnight, with improved somatic awareness.
    • Sleep hygiene: Sleep latency has reduced from 2 hours to 30 minutes via consistent stimulus control.
    • Workplace boundaries: Successfully negotiated a Temporary Adjusted Return to Work plan, ceasing after-hours email monitoring.

While acute anxiety has stabilised, Ms Jenkins continues to experience anticipatory worry regarding an upcoming performance review and exhibits residual perfectionistic traits.

Treatment has utilised Cognitive Behavioural Therapy (CBT) focusing on cognitive restructuring of catastrophic thoughts, alongside Acceptance and Commitment Therapy (ACT) defusion techniques and progressive muscle relaxation.

Sarah Jenkins has attended 6 sessions to date. Based on their progress, I recommend Sarah Jenkins continue receiving psychological support. Could you please provide Sarah Jenkins with a new referral letter for four additional sessions so they can continue accessing treatment.

Should you need to discuss Sarah Jenkins's psychological care, I would be pleased to speak with you.

    "Kind regards,"
