SOAP Note (Psychodynamic/Object-relations)
Subjective
- Pt (Elena Vance, 34F, DOB: 14/05/1990, MRN: 849-201-44) reports recently receiving a promotion to Senior Project Manager on 01/11/2024.
- Describes feeling alienated from her partner, Mark, perceiving him as increasingly critical and emotionally unavailable since her promotion.
- Acknowledges she was able to pause before reacting defensively during a recent disagreement with Mark, noting this as a shift from prior explosive arguments.
- Reports a persistent fear of being exposed as an "impostor" at work and an underlying belief that others inevitably abandon her once she achieves independence.
- Experienced acute anxiety, somatic tension, and tearfulness when recounting her mother's tepid reaction to her career news.
- Demonstrates a recurring pattern of over-functioning in professional settings while preemptively withdrawing from romantic intimacy to avoid perceived rejection.
- Triggered on 05/11/2024 at 18:30 when Mark postponed their celebratory dinner due to a work emergency, which she immediately interpreted as deliberate Sabotage.
- Recognizes that her immediate anger toward Mark closely mirrors the helplessness she felt as a child seeking validation from her narcissistic father.
Objective
- Pt demonstrates emerging intellectual insight into how early parental object relations shape her current expectations of her partner.
- Capacity for self-reflection is moderate; able to mentalize her own states when calm, though this capacity diminishes rapidly under high affective arousal.
- Affect was constricted initially, shifting to tearful and congruent when discussing maternal attachment, then stabilizing by session end. Denies SI/HI.
- Thought process linear, logical, and goal-directed; intact associations; no perceptual disturbances.
- Showing measurable progress in tolerating ambivalent feelings toward love objects without immediately resorting to behavioral acting out.
Assessment
- Focus remains on resolving insecure attachment patterns and integrating split object representations (idealized vs. persecutory).
- Core conflict stems from early emotional neglect and conditional maternal validation, driving a fear that personal success equates to loss of love.
- Relies primarily on projection (attributing her own harsh superego critiques to Mark) and splitting when feeling vulnerable, though use of repression is decreasing.
- Making steady progress toward the goal of reducing interpersonal reactivity and establishing a more cohesive, resilient sense of self.
- Developing greater ego strength and an improved capacity to observe her own defensive shifts in real-time during transference enactments.
Plan
- Continue weekly psychodynamic psychotherapy focusing on transference analysis and interpretation of projective identification within her primary relationship.
- Recommend pt practice affect-labeling and mindfulness techniques when noticing the onset of somatic anxiety prior to engaging in conflict with partner.
- Reinforce pt's successful containment of impulsive anger during the recent dinner cancellation incident.
- Ongoing goal: Foster depressive position functioning (capacity for whole-object relations and tolerance of ambivalence).
- Continue escitalopram (Lexapro®) 10 mg PO daily for baseline anxiety management; no side effects reported. Next session scheduled for 19/11/2024 at 14:00 America/Chicago.
