What postcoordination is
Some ICD-11 codes can be qualified with extra detail: which side, how severe, what caused it. Combining a base code with those qualifiers is postcoordination, and it is how ICD-11 records a specific clinical picture rather than an approximate one.

ICD-11 only
Postcoordination is a feature of ICD-11. It does not apply to SNOMED or to your own uploaded standards.
Only what is documented
Add a qualifier because the record supports it, not because the axis exists. An unsupported qualifier is a coding error in exactly the same way a wrong base code is.
