Therapeutic Interventions
The clinician utilized Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) in accordance with the Cohen, Mannarino, and Deblinger model during the 50-minute session on 24/10/2023 at 14:00.
Specific methods included introducing the cognitive triangle to facilitate cognitive coping and cognitive restructuring of trauma-related maladaptive cognitions. Socratic questioning and interactive everyday scenarios were employed to challenge the patient's inaccurate beliefs regarding self-blame. To manage autonomic hyperarousal during gradual exposure to trauma reminders, the clinician guided the patient through relaxation skills, specifically diaphragmatic breathing, progressive muscle relaxation (PMR), a 5-4-3-2-1 mindfulness grounding practice, and safe-place visualization, supplemented by bilateral rhythmic movement and calming instrumental music.
These interventions correspond directly to the initial stabilization and skill-building phases of the PRACTICE acronym, establishing affective modulation and cognitive coping skills necessary to support subsequent trauma narrative processing and in vivo mastery.
Assessment of Patient's Response to Therapeutic Interventions
The patient, a 14-year-old female (MRN: 849-302-11), demonstrated high engagement and active participation throughout the session.
She successfully demonstrated accurate identification of interconnected thoughts, feelings, and behaviors using the cognitive triangle, alongside independent execution of paced diaphragmatic breathing.
She responded receptively to Socratic questioning and guided visualization, reporting a decrease in Subjective Units of Distress Scale (SUDS) from 7/10 to 3/10 following the PMR exercise.
Marked improvement in self-awareness and cognitive flexibility was evident as she generated alternative, realistic replacement thoughts to counter automatic self-blaming cognitions.
Physiologically, her baseline psychomotor agitation and shallow tachypnea resolved into relaxed posture and normopnea during mindfulness grounding.
Enhanced metacognitive ability and emotional self-regulation were observed when she paused to evaluate the validity of an intrusive trauma-related thought rather than reacting with affective dysregulation.
Her overall mood was euthymic with a congruent, increasingly optimistic outlook regarding trauma recovery.
She reported successfully applying controlled breathing and grounding strategies at school earlier in the week when confronted with an unexpected auditory trauma trigger.
Positive behavioral and cognitive shifts included reduced avoidance of social interactions with peers and a verbalized reduction in pervasive guilt.
She expressed strong willingness to incorporate daily PMR and thought-log tracking into her evening routine alongside her maintenance sertraline (Zoloft®) 50 mg PO QD regimen.
This progress aligns with core TF-CBT treatment goals, indicating readiness to transition from the stabilization phase into formal trauma narrative development and processing.
