    Dear Sarah Jenkins,

Thank you for coming to our clinic on 24/05/2024. We were glad to help with your health needs. Here is a brief summary of the main points covered at your visit:

    • You came in today because your right knee has been hurting for the past three weeks. The pain gets worse when you walk up stairs or stand for a long time. We examined your knee and talked about how gentle wear and tear on the joint is likely causing the soreness.
    • You have a history of high blood pressure. You also had an operation to remove your appendix ten years ago.
    • You currently take amlodipine 5 milligrams by mouth once every day for your blood pressure, and a vitamin D supplement of 1,000 international units by mouth once every day.
    • You live in an apartment with your husband and work at a desk job. You do not smoke cigarettes and you drink one glass of wine on the weekends.
    • You are allergic to penicillin, which gives you an itchy skin rash.

At the visit, we also went over these recommendations and next steps

    • To help with your knee pain, you can take acetaminophen (Tylenol) 500 milligrams by mouth every six hours when needed for pain. Try to rest your knee when it hurts and put an ice pack on it for fifteen minutes after walking. We also gave you a sheet of gentle leg exercises to make the muscles around your knee stronger. Please book a follow-up visit to see us again in four weeks if your knee is not feeling better, or sooner if your knee becomes very swollen, red, or hot to the touch.

Please contact our office if you have questions or would like anything explained further. We are here to help you work towards your health goals.

    Sincerely,

Doctor Emily Chen Family Medicine Specialist Pacific Spirit Family Practice 2150 West Broadway, Vancouver, British Columbia, V6K 4L9 | Phone: 604-555-0192
