    Interpretation of Labs

    • Fasting metabolic panel and lipid profile from 14/05/2026 demonstrate suboptimal glycemic control and persistent dyslipidemia. Renal function and electrolytes are WNL.
    • HbA1c elevated at 8.2 %, fasting blood glucose elevated at 168 mg/dL, LDL-C elevated at 142 mg/dL, and triglycerides elevated at 210 mg/dL.
    • Progressive upward trend in HbA1c and LDL-C over the past 12 months.
    • Compared to labs from 10/11/2025, HbA1c increased from 7.4 % to 8.2 % and LDL-C rose from 128 mg/dL to 142 mg/dL. eGFR remains stable at > 60 mL/min/1.73 m².

    Assessment

    • Uncontrolled T2DM and mixed hyperlipidemia, with preserved renal function.
    • Type 2 diabetes mellitus without complications (hyperglycemia), mixed hyperlipidemia.
    • Worsening glycemic and lipid parameters increase long-term ASCVD and microvascular risk, warranting immediate pharmacotherapy intensification.

    Plan

    • Intensify oral hypoglycemic and lipid-lowering regimens; reinforce lifestyle modifications.
    • Repeat HbA1c, BMP, and fasting lipid panel in 3 months (14/08/2026). Obtain annual urine albumin-to-creatinine ratio (UACR) at next visit.
    • Increase metformin (Glucophage®) from 500 mg PO BID to 1,000 mg PO BID. Increase atorvastatin (Lipitor®) from 20 mg PO daily to 40 mg PO daily.
    • Counseled on low-carbohydrate, low-saturated-fat diet and target of 150 minutes/week of moderate aerobic exercise. Discussed potential GI side effects of increased metformin dose and signs of statin-associated myopathy.
