Subjective
Arthur Pendelton (DOB: 14/05/1968, PHN: 9876-543-210) presents today on 19/10/2023 for evaluation of progressively worsening shortness of breath, fatigue, and a persistent low mood over the past 4 weeks. Arthur reports dyspnoea on exertion after walking 100 metres on flat ground and notes new bilateral ankle swelling. He has been sleeping poorly due to needing two pillows to breathe comfortably (orthopnoea) and waking up gasping for air twice in the last week. Additionally, Arthur states he has felt increasingly down and anxious about his declining health and recent job loss, though he explicitly states he has no suicidal ideation, homicidal ideation, or intent for self-harm.

Assessment & Plan
- Suspected New-Onset Heart Failure
- Assessment: Arthur presents with classic symptoms and signs of volume overload, including exertional dyspnoea, orthopnoea, PND, elevated JVP, bibasilar crackles, S3 gallop, and pedal oedema, alongside a 3 kg weight gain. Given his history of hypertension and T2DM, heart failure (either preserved or reduced ejection fraction) is the primary working diagnosis. He is haemodynamically stable enough for outpatient management today.
- Differential diagnosis:
- Heart failure with reduced or preserved ejection fraction (HFrEF/HFpEF)
- Chronic kidney disease / nephrotic syndrome
- Undiagnosed COPD or interstitial lung disease
- Silent myocardial ischaemia
- Investigations:
- Urgent blood work: FBC, electrolytes, urea, creatinine, eGFR, fasting lipid profile, HbA1c, TSH, LFTs, and NT-proBNP.
- Urine dipstick and urine albumin-to-creatinine ratio (ACR).
- Chest X-ray (PA and lateral) ordered today.
- Transthoracic echocardiogram (TTE) requested urgently.
- Treatment:
- Initiate furosemide (Lasix®) 40 mg PO daily for diuresis.
- Continue ramipril 10 mg PO daily and atorvastatin 20 mg PO daily.
- Restrict dietary sodium to < 2,000 mg/day and fluid intake to 1.5–2 L/day.
- Daily morning weights at home; advised to call clinic if weight increases by > 1.5 kg in 2 days.
- Referrals:
- Cardiology outpatient clinic for heart failure assessment and optimisation.
- Follow-up:
- Review in clinic in 1 week (26/10/2023) with repeat electrolytes and renal function panel to monitor furosemide response.
- Red flags discussed: instructed to present to the nearest Emergency Department immediately if experiencing worsening shortness of breath at rest, chest pain, or syncope.
- Adjustment Disorder with Depressed and Anxious Mood
- Assessment: Arthur reports low mood and anxiety secondary to recent unemployment and declining physical health. MSE confirms reactive mood changes with intact insight and judgement, and no acute safety concerns (explicitly no suicidal ideation or self-harm intent). Symptoms are partly compounded by poor sleep from orthopnoea.
- Differential diagnosis:
- Adjustment disorder with mixed anxiety and depressed mood
- Major depressive disorder
- Insomnia secondary to medical condition (orthopnoea/heart failure)
- Treatment:
- Non-pharmacological management initiated first; sleep hygiene and behavioural activation discussed.
- Defer antidepressant therapy today pending diuresis and improvement of nocturnal respiratory symptoms.
- Provided Crisis Line contact details as a standard safety precaution.
- Referrals:
- Referred to the Primary Care Network (PCN) Behavioural Health Consultant for cognitive behavioural therapy (CBT) and employment stress support.
- Follow-up:
- Reassess mood and sleep at the 1-week follow-up appointment, and formally score PHQ-9 and GAD-7 in 4 weeks.
