    Therapy Session

    Subjective + Observations

Leo (DOB: 12/05/2020) transitioned into the clinic room independently and separated from his mother without distress. He appeared well-rested, alert, and immediately engaged with the toy farm set on the floor.

    Session Goals

    • Produce /k/ and /g/ in initial word position at the phrase level with 80% accuracy.
    • Follow 2-step unrelated directions containing spatial concepts (in, on, under) during structured play.

    Parent Report

Mother reported Leo has been practising his "coughing sound" (/k/) at home using the provided flashcards ~3x this week. She noted he is starting to self-correct when saying "car" and "cup" in spontaneous speech, though still substitutes /t/ for /k/ when excited or speaking quickly.

Activities - Informal observations and SP recommendations

    • Pop-up Pirate & Articulation Cards: Targeted initial /k/ and /g/ in 2-3 word carrier phrases ("put in [card]"). Leo achieved 75% accuracy for /k/ and 80% accuracy for /g/ with minimal tactile-visual cueing (pointing to throat).
    • Obstacle Course / Farm Play: Targeted 2-step directions with spatial concepts. Leo completed 4/5 trials accurately with repetition of instructions required on 2 occasions due to distraction.

Discussed shifting home practice from single words to short carrier phrases (e.g., "my car", "big cup"). Recommended modelling correct production naturally via recasting rather than asking him to repeat words during conversational speech to avoid frustration.

Maintained good attention for the first 25 mins of the session. Became slightly restless during the final tabletop activity; benefited from a movement break and a visual schedule to complete the final task.

    Plan

R/V in 2 weeks (24/05/2024 at 10:00). Continue targeting velar fronting at phrase/sentence level and introduce temporal concepts (before/after) in receptive language tasks.
