Referred for: Left groin swelling
Diagnosis: Left indirect inguinal hernia
Plan: Elective left inguinal herniotomy
Thank you for referring Leo Vance (DOB: 14/05/2022, MRN: 492-810-33), a 2-year-old male presenting with an intermittent left inguinoscrotal bulge noted by his parents over the past 3 weeks, primarily during crying or straining. No history of vomiting, irritability, or irreducible episodes.
O/E: Alert, active, NAD. Abdomen soft, non-tender. Left groin demonstrates a palpable, reducible indirect inguinal hernia with a positive silk glove sign. Right groin NAD. Bilateral testes descended and normal in size/consistency.
Surgical repair is indicated to prevent incarceration and subsequent bowel strangulation or ipsilateral testicular ischaemia. Timely elective repair eliminates this acute risk and avoids emergent intervention.
We have scheduled a day-case open left inguinal herniotomy under GA with a caudal block. Post-op analgesia will be managed with paracetamol (Panadol®) 240 mg PO QID PRN and ibuprofen (Nurofen®) 160 mg PO TID PRN.
Parents were provided with the Women's and Children's Hospital consumer health handout on paediatric inguinal hernia repair and clear red-flag return precautions (irreducible mass, erythema, bilious emesis).
Thank you again for this referral; I will forward the operative notes and discharge summary following his procedure on 28/11/2024.
