Participant Name: Liam O'Connor Date of Case Note: 24/05/2024 Support Coordinator: Sarah Jenkins Type of Contact: In-Person
-
Purpose of Contact The Support Coordinator met with Liam to conduct a scheduled monthly review of his NDIS supports, monitor his Capacity Building budget utilisation, and address recent challenges regarding community access and psychology engagement.
-
Key Discussion Points Liam reported increased social isolation and heightened anxiety over the past three weeks following the departure of his regular support worker. He expressed dissatisfaction with the replacement workers provided by his current agency, noting inconsistent arrival times. The Support Coordinator and Liam reviewed his NDIS goals related to community participation and emotional regulation. They also discussed the progress of his Functional Capacity Assessment (FCA) with his Occupational Therapist (OT) and the need to re-engage with his clinical psychologist for ongoing psychosocial support.
-
Actions Taken The Support Coordinator contacted the Core support provider to formally address the scheduling inconsistencies and requested a dedicated roster of two consistent support workers. A referral was submitted to a new registered psychology clinic with immediate availability for telehealth and in-person sessions. Additionally, the Support Coordinator followed up with the OT to confirm the completion date for the FCA report prior to the upcoming plan reassessment.

Financial Summary & Plan Management Details
Budget Overview
Support Category: Capacity Building - Improved Daily Living Budget Allocated: $15,400.00 Amount Spent: $9,240.00 Remaining Balance: $6,160.00 Notes/Observations: Budget utilisation is currently tracking at 60% with four months remaining in the plan, which is sustainable and sufficient to cover the remaining OT hours, FCA report writing, and fortnightly psychology sessions.
-
Participant Response & Engagement Liam engaged well throughout the meeting, maintaining focus and clearly articulating his preferences for support worker traits. He demonstrated a clear understanding of his remaining budget and expressed relief upon learning that an alternative psychology provider had been sourced. He agreed with the proposed plan to trial a new support worker next week.
-
Risks / Issues Identified There is a moderate risk of mental health deterioration and further social withdrawal if consistent Core supports and psychological therapies are not re-established promptly. No immediate safety risks or housing instability issues were identified during the visit.
-
Recommendations & Plan Adjustments The Support Coordinator recommended shifting two hours per week from weekend community access to weekday morning routines to assist Liam with his sleep-wake cycle and medication compliance (sertraline 100 mg PO daily). It was also recommended that the upcoming FCA report be used to request an increase in Specialist Support Coordination hours at the next plan reassessment due to complex service navigation needs.
-
Additional Notes / Observations Liam's apartment was observed to be clean and well-maintained. His sister, Chloe (informal support), continues to visit every Sunday to assist with meal preparation and grocery shopping, which provides stable informal support.
-
Next Steps & Follow-Up The Support Coordinator will email the new psychology service agreement to Liam for signing by 27/05/2024. The Support Coordinator will also follow up with the Core provider on 31/05/2024 to review the outcomes of the shadow shifts with the new support workers, and will schedule the next monthly check-in with Liam for 21/06/2024 at 10:00.
