Subjective
- The patient presents with a 3-week history of right-sided lower back pain rated at 6/10 in intensity, characterized as a constant dull ache with intermittent sharp catches.
- The onset was insidious following a weekend of heavy gardening, and the pain is localized to the right lumbosacral region with occasional referral into the right superior gluteal area. Symptoms are aggravated by prolonged sitting (> 30 minutes), forward flexion, and transitioning from sit to stand, while gentle walking and heat application provide temporary relief.
- The patient denies any lower limb paresthesia, numbness, weakness, or saddle anesthesia, and reports normal bowel and bladder function.
- Past medical history:
- Right knee arthroscopy (2018)
- Mild asthma
- Maternal history of osteoarthritis
- The patient works full-time as an accountant requiring prolonged desk sitting, and normally walks 3 km three times per week, which has ceased due to the current pain.
- Current medications and treatments:
- Ibuprofen (Nurofen®) 400 mg PO TID PRN
- Salbutamol (Ventolin®) 100 mcg INH PRN
- Magnesium glycinate 300 mg PO OD
- Heat pack application daily
- The patient lives in a single-story house with his partner, reports supportive home circumstances, and notes mild work-related stress during the current financial quarter.
- Allergies:
- Penicillin (rash)
- Sleep is disrupted 2 to 3 times per night when rolling over in bed, though the patient denies snoring or mouth breathing.
Objective
- BP 124/78 mmHg, HR 68 bpm, RR 14 breaths/min, Temp 36.7 °C.
- Structural observation reveals a mild flattening of the lumbar lordosis and an elevated right iliac crest in standing, though gait remains non-antalgic. Lumbar active range of motion is restricted in flexion to mid-shin (reproducing right lumbosacral pain) and right lateral flexion is reduced by 50%, while palpation identifies hypertonicity in the right quadratus lumborum and lumbar erector spinae, alongside restricted posteroanterior accessory glide at the L4/L5 and L5/S1 facet joints.
- Lower limb neurological examination is unremarkable with patellar (L4) and Achilles (S1) reflexes 2+ and symmetrical, myotomes L2-S1 graded 5/5 bilaterally, intact sensation to light touch across all lower limb dermatomes, and a negative straight leg raise bilaterally.
- Lumbar spine X-ray (dated 10/05/2026) demonstrates mild disc space narrowing at L4/L5 and early facet joint arthropathy at L4/L5 and L5/S1, with no evidence of spondylolisthesis or fracture.
Assessment
- The patient presents with acute-on-chronic mechanical low back pain secondary to right L4/L5 and L5/S1 facet joint somatic dysfunction and associated myofascial hypertonicity.
- Key areas for treatment include the right lumbar paraspinal musculature, right quadratus lumborum, and right L4-S1 articular pillars to restore segmental mobility, reduce nociceptive input, and normalize lumbopelvic biomechanics.
Plan
- The treatment plan incorporates soft tissue massage and myofascial release to the right lumbar paraspinals and quadratus lumborum, muscle energy techniques (MET) to the lumbar spine, and Grade III-IV joint mobilization to the right L4/L5 and L5/S1 facet joints.
- The patient will attend physiotherapy twice weekly for the initial 2 weeks, tapering to once weekly for a further 3 weeks depending on clinical progression.
- Short-term goals are to reduce baseline pain to ≤ 2/10 and restore pain-free sitting for 60 minutes within 2 weeks, while long-term goals include returning to full gardening and walking activities without limitation within 6 weeks.
- The patient is advised to utilize a lumbar support roll at his office desk, take micro-breaks to stand and walk every 30 minutes during work hours, and commence a gentle home mobility program.
- No further diagnostic imaging is indicated at this stage, and a summary letter will be sent to the patient's general practitioner if symptoms fail to improve within 3 weeks.
Interventions
- Soft tissue massage and myofascial release were applied to the right lumbar erector spinae, quadratus lumborum, and gluteus medius for 10 minutes, followed by Grade III posteroanterior mobilizations to the right L4/L5 and L5/S1 facet joints (3 sets of 60 seconds) and lumbar rotation MET.
- Following treatment, the patient reported an immediate reduction in resting pain to 3/10, and reassessment demonstrated a 25% improvement in active lumbar flexion and right lateral flexion with reduced end-range stiffness.
Evaluation
- This is the initial consultation; however, the patient demonstrated a positive immediate response to manual therapy with improved lumbar excursion and decreased resting pain.
- The current plan will be maintained, with the planned addition of isometric core stability exercises at the next session if the acute pain settles further.
Additional Notes
- The patient was educated on the benign nature of mechanical low back pain, the importance of avoiding prolonged static postures, and pacing strategies when returning to physical tasks.
- A home exercise program was provided consisting of supine pelvic tilts (2 sets of 10 repetitions), knee-to-chest stretches (3 repetitions held for 30 seconds each), and cat-camel mobility exercises to be performed twice daily.
- The patient expressed a preference to avoid high-velocity low-amplitude (HVLA) thrust manipulations to the lower back, which has been documented and respected in the ongoing treatment plan.
