Re: Sarah Jenkins (DOB: 12/08/1988) - Generalised Anxiety Disorder and Moderate Depressive Episode
Dear Dr. Marcus Vance,
Thank your ongoing care of Sarah Jenkins whom I saw today for review via Telehealth. Today constituted our 6th session under her current Better Access Mental Health Treatment Plan. Sarah reported a gradual improvement in mood and a reduction in somatic anxiety symptoms following the commencement of sertraline (Zoloft®) 50 mg PO daily four weeks ago, alongside ongoing CBT. Her K10 score today was 22, down from 30 at baseline. We focused on cognitive restructuring for workplace performance anxiety and consolidating sleep hygiene strategies. On MSE, she was euthymic, with reactive affect, logical thought form, and nil suicidal ideation or intent.
Plan
- Continue fortnightly Telehealth psychology sessions focusing on CBT and graded exposure to workplace stressors.
- Patient to maintain daily thought records and practice progressive muscle relaxation (PMR) prior to sleep.
- GP review requested following this 6th session for a Mental Health Treatment Plan (MHTP) review to access a further 4 rebated sessions.
- Continue current pharmacotherapy regimen as prescribed by you.
