About this template
A structured template for documenting the initial emergency trauma evaluation, including pre-arrival information, EMS hand-off, trauma team details, and the ABCDE primary survey. It captures vital signs, airway, breathing, circulation, neurological and cervical spine findings, exposure, and major interventions.
