    • Reason: Evaluation of Class II malocclusion and mandibular crowding.

Presenting Complaint / Reason for Consultation

    • Pt reports dissatisfaction with "crooked bottom teeth" and prominent upper incisors. Requests discreet orthodontic tx, expressing strong preference for Invisalign over fixed appliances.

    Dental and Orthodontic History

    • Regular 6-month dental recall. Last hygiene visit 04/09/2024.
    • Uncomplicated extractions of deciduous teeth; no permanent extractions.
    • No prior orthodontic tx.

    Medical History

    • Mild intermittent asthma.
    • Meds: salbutamol (Ventolin®) 100 mcg INH PRN.
    • Allergies: NKDA, no latex allergy.

    Social and Family History

    • High school student; plays flute in school band.
    • Non-smoker, no alcohol.
    • Brushes BID, flosses 2-3x/wk.
    • Father had fixed appliances for Class II malocclusion as a teenager.

    Examination Findings

    • Extraoral: Mesocephalic, symmetrical face, mildly convex profile. Competent lips at rest. TMJ: WNL, no clicks, crepitus, or tenderness.
    • Intraoral: Permanent dentition present 7-7. Oral hygiene fair; mild marginal gingivitis lower anteriors.
    • Arches: U-shaped maxillary and mandibular arches. Mild maxillary crowding (2 mm); moderate mandibular anterior crowding (5 mm).
    • Occlusion: Molar relationship Class II (half-cusp) bilaterally. Canine relationship Class II bilaterally. Overjet 5 mm. Overbite 40% (3 mm). Midlines coincident with facial midline. No crossbites.

    Radiographs and Diagnostic Records

    • OPG (taken 24/10/2024): All 32 teeth present; 18, 28, 38, 48 unerupted, crown formation complete, mesioangular impaction of 38 and 48 likely. Condyles symmetrical. Normal root lengths and alveolar bone levels. No caries noted.
    • Lateral Ceph: Skeletal Class II pattern (ANB 5°), normodivergent vertical pattern, mildly proclined upper incisors.
    • Intraoral/Extraoral photos and iTero digital scan completed.

    Diagnosis

    • Dental Class II division 1 subdivision on a mild Skeletal Class II base, complicated by moderate mandibular crowding and increased overjet.

    Treatment Options Discussed

    • Option 1: Comprehensive Clear Aligner Therapy (Invisalign) with Class II elastics and interproximal reduction (IPR). Est. duration: 18–20 mos. Pros: Esthetic, removable for hygiene and flute playing. Cons: Strictly compliance-dependent (22 hrs/day wear required).
    • Option 2: Fixed Labial Appliances (0.022" slot metal or ceramic brackets) with Class II elastics and mandibular IPR. Est. duration: 18–20 mos. Pros: Not compliance-dependent for tooth movement (except elastics). Cons: Less esthetic, harder hygiene, initial adaptation needed for wind instrument.
    • Option 3: No treatment. Monitor only.

    Recommended Treatment Plan

    • Comprehensive Invisalign treatment (upper and lower arches).
    • Composite attachments on premolars and canines; precision cuts for 1/4" medium Class II elastics.
    • 0.3 mm IPR across mandibular anteriors (33–43) to relieve crowding without excessive proclination.
    • Estimated timeline: 18 months active tx, changed q7d, followed by retention phase (upper and lower Vivera retainers + bonded lingual wire 33–43).

    Risks, Benefits, and Alternatives

    • Risks discussed: Root resorption, decalcification/caries with poor OH, gingivitis, relapse without retainer compliance, temporary speech alteration, TMJ symptoms, failure to achieve ideal occlusion if aligner/elastic wear is < 22 hrs/day (may require transition to fixed braces).
    • Benefits: Improved esthetics, alignment, and occlusal function.
    • Alternatives: Fixed appliances or no treatment (crowding may worsen with age).

Patient / Guardian Questions and Preferences

    • Pt and mother (Sarah MacDonald) asked about pain levels and impact on playing the flute. Reassured that aligners can be removed briefly for performances if needed, and mild soreness is managed with paracetamol (acetaminophen, Tylenol®) 500 mg PO PRN.
    • Both opted to proceed with the Invisalign plan.

    Consent

    • Verbal and written informed consent obtained from mother (legal guardian) and assent from patient. Financial contract reviewed and signed.

    Next Steps and Follow-Up

    • ClinCheck setup to be finalized by orthodontist.
    • RTC in 4 weeks for attachment bonding, IPR, and delivery of initial aligners.
    • Recommend wisdom teeth (18, 28, 38, 48) reassessment for extraction in 2 years.

    Patient/Guardian Education

    • Instructed on increasing flossing frequency to daily prior to appliance delivery.
    • Reviewed dietary restrictions with aligners: water only while trays are in; remove for all food and caloric/acidic beverages, brush before reinsertion.
    • Emphasized critical role of 22 hrs/day wear time for both trays and elastics.
